COVID-19 CONTACT TRACING SUBMISSION

Please fill out our contact tracing form prior to renting our tour equipment's or participating in our activities. Contact tracing will be used to track possible participants exposures in case of COVID-19 outbreak. Proof of COVID-19 vaccination is mandatory requirement to participate in our activities. Please present proof of COVID-19 vaccination passport together with your government issued photo identification card/drivers license or passport to your tour guide prior to attending activities. For expediated process you can also email us in advance of your tour date. When sending proof of vaccination by email, please include your booking reference number in email subject line.

I/we have not been diagnosed or cared for anyone with COVID-19 in the past 2 weeks

I/we have not shown signs or been in close contact with anyone that is exhibiting these symptoms: COUGH, FEVER/CHILLS, SHORTNESS OF BREATH, DIFFICULTY BREATHING, SORE THROAT, LOSS OF TASTE OR SMELL, FATIGUE, HEADACHE, CONGESTION, RUNNY NOSE, NAUSEA, VOMITING OR DIARRHEA.

I'm/we're not caring for someone self isolating or been asked by the Office of the Chief Public Health Officer (OCPHO) to isolate or ordered to get tested for COVID-19.

If I/we begin to show symptoms of COVID-19 within the next 10 days I will contact Arctic Tours Canada.

I/we will follow Arctic Tours Canada and the Office of the Chief Public Health Officer (OCPHO) rules to keep myself/ourselves, my/our guide, and those around me/us safe.
